Summary: We present a case of open aortic reconstruction for a 15-year-old patient with mid-aortic syndrome and renovascular hypertension. The procedure involved a complex aorto-aortic bypass with renal and mesenteric artery reconstructions, performed through a transperitoneal approach through a aorto-aortic graft and pre-fashioned revascularization of the superior mesenteric artery and bilateral renal arteries. Postoperatively, the patient demonstrated excellent recovery with significant improvement in blood pressure control and graft patency confirmed at follow-up.
